This service includes a one-hour, in person assessment that will result in a care plan with resources and the best next steps to keep your loved one safe and healthy.
Coordinating and Monitoring of Medical Services
This includes an assessment, care plan development and outline for continued support. This option is best for seniors with limited local support. Ambassador can be your advocate and ensure quality care services are being provided while communicating updates with distant loved ones.
Senior Living and Home Care Referrals
This includes an initial consultation, assessment and care plan development to determine the best next steps with home care or senior living. Guidance and support throughout this process includes assistance with scheduling tours and meetings with senior living communities and helping families to determine the best option.
Dementia and Alzheimer's Support
Receiving a dementia diagnosis can leave a lot of unanswered questions. This is the right time to lean on others for assistance and support. Ambassador can offer that support, guidance and resources that can lead you to a better peace of mind.
Advocacy
Do you need to have a difficult conversation with a loved one about needing more help? Does your loved one live in a senior living community and you have questions surrounding the care they are receiving? Ambassador can be your advocate and help to initiate and facilitate those difficult conversations.
